Memory Clinic

Program Type: Mental Health & Well-being

When a patient or a family member starts to recognize memory changes in themselves or their loved one, the individual will have an opportunity to receive a comprehensive assessment by an interdisciplinary team of professionals that are part of the STAR Family Health Team Memory Clinic. The team is comprised of a physician, a registered nurse, a social worker, and very supportive members of community agencies which include CCAC and the Alzheimer Society of Perth County.

After receiving a referral to the clinic by the family physician, the patient can expect assessments on many aspects of memory function including driving safety. The patient and family will receive recommendations on treatment options and plans for care. Our goal is to help patients with memory difficulties maintain independent living and best quality of life for as long as possible.

The Memory Clinic is offered monthly rotating between the three sites of the STAR Family Health Team.

Mending Through Mindfulness - MBCT (Mindfulness Based Cognitive Therapy)

Program Type: Mental Health & Well-being

What is Mindfulness Based Cognitive Therapy (MBCT)?

MBCT helps reduce stress and anxiety and prevents relapses in those who suffer from depression. It combines the practice of mindfulness meditation with the practical skills of learning to interrupt thought patterns that lead to negative spirals in mood. Mindfulness is a non-judgmental way of paying attention in the present moment and leads to increased personal awareness and clarity.

Program Description

Offered at no cost to patients of the STAR Family Health Team, this 8-week program is for anyone experiencing moderate anxiety/depression. It is also highly recommended for people with a history of depression.

Participants will be trained to identify the triggers and symptoms of anxiety and depression, and then effectively address them as they arise in everyday life.

Online registration is not available for this program. Please contact Amanda at 519-655-2322 for more information or to register. Thank you.
