Description:
We are looking for a contract Nurse Practitioner to work collaboratively with all members of our multidisciplinary team. You will provide comprehensive acute or episodic primary care and chronic disease management to a variety of patients. Experience with procedures is an asset. You will also assist in the development of programs, resources and the creation of linkages within the Family Health Team.
Qualifications:
You will have current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class and evidence of professional liability insurance. You will be located primarily in Stratford. Experience with Microsoft Office and EMR using both Apple and PC computers will be required. With an ability to work as a professional member of a multidisciplinary team, the Nurse Practitioner must be self-directed, motivated, and demonstrate flexibility and organization to manage time effectively in this busy clinic environment.
