Land Acknowledgement

We acknowledge and give thanks for the land on which we gather as being the traditional territory of the Haudenosaunee/People of the Long House and the Anishinaabe.

We recognize the First Peoples’ continued stewardship of the land and water, and that this territory is subject to the Dish with One Spoon Wampum under which multiple nations agreed to care for the land and resources by the Great Lakes in peace.

We also acknowledge and recognize the treaties signed in regard to this land including Treaty #29 and Treaty #45 1⁄2. Use of Allied Health Professionals

At the STAR Family Health Team we offer services from a variety of allied health professionals to ensure you get the care you need. These include:

Nurse Practitioners

A Nurse Practitioner (NP) provides care to patients of all ages with a focus on preventing disease, promoting good health and curing illness. Nurse Practitioners play an important role in helping you to maintain a healthy, active lifestyle, and to understand and/or treat your illness.

 

As a member of your health care team, NP’s offer a variety of health care services including the ability to: 

  • diagnose and treat illness and/or injuries,
  • perform physical check-ups, 
  • order and interpret diagnostic tests, 
  • write prescriptions, 
  • provide counselling and education, 
  • provide supportive care through illness, 
  • provide treatments and/or procedures, and 
  • make referrals to family physicians, specialists and other health professionals
  • and more!
Pharmacists
The Pharmacist is “a person prepared to formulate, dispense, and provide clinical information on drugs or medications to health professionals and patients.” As a member of the health care team pharmacists play a key role in providing quality healthcare and pharmaceutical care to the public. (WHO)
 
Registered Pharmacists (RPh) either have a Bachelors of Science (BScPHM) or a Doctor of Pharmacy (PharmD) degree. They are registered with the Ontario College of Pharmacists (OCP) their licensing body. 
 
Pharmacists for STAR FHT provide healthcare in the following areas:
 
  • Smoking cessation (STOP Program)
  • COPD Clinics
  • Diabetes Clinics
  • Insulin starts and titration
  • Medication chart reviews and deprescribing
  • Bone Health
  • Pain management
  • Clinical consultations
  • Education of healthcare team through newsletters and education sessions
  • Patient consults 
  • Medication review for Memory clinics

Registered Dietitians

A Registered Dietitian (RD) is a regulated health professional with a Bachelor’s degree specializing in food and nutrition, who has also completed an internship program and passed a national exam. Some RDs also have a Master’s degree in nutrition.

To practice in Ontario, RDs must be registered with the College of Dietitians of Ontario.

RDs are experts in nutrition, and can provide you with advice, education, and plans that relate to both food and a healthy lifestyle.

Here are some of the things you can speak with a Registered Dietitian about:

  • What foods to eat to lower your risk or manage conditions such as diabetes, high blood pressure, high cholesterol, etc.
  • The best way to achieve and maintain a healthy weight for your body.
  • Individualized eating plans for medical conditions such as food allergies or intolerances, gastrointestinal conditions such as Irritable Bowel Syndrome, Celiac, Crohn’s and more.
  • Meal planning for a busy lifestyle.
  • Eating for a healthy pregnancy.
  • Introducing solids to your baby.
  • Tips for picky eaters
  • What vitamin/mineral supplements you might need.
  • How to modify your diet after a medical procedure, like surgery.
  • How to read food labels.
  • How to be a healthy vegetarian or vegan.

Mental Health Clinicians

The STAR Family Health Team counsellors are registered mental health professionals who are trained and authorized to practice Psychotherapy. Our team is made up of Registered Psychotherapists, Social Workers and a Psychological Associate. Who you see is determined by a number of factors, including where you live and the reason you are coming for counselling.

Counsellors are able to see patients with a wide range of mental health-related problems. They can work with individuals (children, teens, adults and seniors) and families.

Common reasons for referral to the counsellor include:

  • depression

  • stress or anxiety

  • relationship issues

  • divorce/separation

  • trauma

  • bereavement/loss/grief

  • work-related problems

  • parenting support

  • behaviour/learning difficulties

  • identity issues

What is a Family Health Team?

A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient. Our team consists of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills within the clinic to provide you with the very best and most up-to-date care, when you need it.

How will the Family Health Team benefit me?

The services focus on primary health care including disease prevention, health promotion and disease management.

Will I still get to see my doctor if he or she joins a Family Health Team?

Yes. Family Health Teams are a team approach to health care where your doctor will be able to focus on your complex medical problems but you’ll have access to different health care professionals according to your needs.

Are there After Hours Clinics?

Yes. After hours care is available evenings Monday-Thursday and Saturday mornings by appointment only for rostered patients of STAR Family Health Team. Information regarding after hour clinics is available here.

 

Will I always see my family physician at my appointments?

Not necessarily. Sometimes you may see another member of our interdisciplinary team such as a nurse practitioner, mental health clinician or registered dietitian within your circle of care.
